To Obtain A Replacement Card

If you have lost or damaged your card, you must provide the following information when making a request::

1. Your First and Last Name
2. The Date you took the class
   (or an estimated time frame of when the class was taken, the more precise, the better.)
3. The Type of Class you took *
4. Where you took the Class *
5. The Instructor's Name *
6. A self-addressed stamped envelope
7. There is a $10.00 fee that must be paid before a replacement card is typed.

* Note: Marked items are not required, but by providing as much information as possible, you help us to serve you better.

For security reasons, all requests will be verified in the Training Center's records.

 

Requests are not accepted via email. For the fastest response when making a request,

Keep in mind that we must be able to read your handwriting in order to verify your request. Illegible requests are unlikely to be verified accurately.
